If you find yourself in a position where you need to replace a lost, stolen, or damaged Medicare card the process can be quite simple. Go to www.ssa.gov/myaccount and sign in to your my Social Security account. If you do not have an account now is a good time to create one.
Once you have logged in with your secure login and password select the tab that says "Replacement Documents". Then you will select "Mail my replacement Medicare card." Be sure to write down your username and password in a secure location so you have it next time you need to login.
It can take up to 30 days before you will receive your physical card in the mail to the address that is on file with Social Security. If your address is not correct with Social Security you can put in a request for it to be changed.
If you need your Medicare ID number before you receive your physical card you can access your Benefit Verification Letter through your my Social Security account. The letter will have your ID number and effective dates on it. If you have a www.Medicare.gov account you can log in to that account and print an official copy of your card. Keep in mind that not all agencies can use a printed copy and would need an original copy, but in many scenarios, a print copy would suffice.
If you cannot sign up for an account, or prefer not to, you can call 1-800-MEDICARE (1-800-633-4227 or TTY 1-877-486-2048) and the representative on the phone can assist you in a request for a duplicate card. You can also visit or call your local Social Security office to report a lost, damaged, or stolen card.
Your Medicare card is important to keep protected and secure. Do not share this number with anyone who you do not know and trust. If you get a call from someone you do not know asking for your Medicare number or a bank account number, or any personally identifiable information, do not release that information to them, unless you requested assistance from them. Medicare and Medicaid (known as CMS), the IRS, and the Social Security Administration will NOT call you asking for this information from you.
